The survey aims to find out about the experiences of NDIS participants during emergency and disaster situations, such as bushfire, flood and the Covid Pandemic. Your responses will help us to give feedback to the NDIS Quality and Safeguards Commission about providing support to NDIS Providers during times of emergency and disaster. They will also help with developing a toolkit of resources for NDIS Providers.
The survey should take about 20 minutes to complete. If you have a google account and are signed in you take a break at any time, Google Forms automatically saves your progress for 30 days when you’re signed in to a Google account.
Your participation in this survey is voluntary. All of your responses will be kept confidential, and will not be linked to any personal demographic information asked in the first section.
